Job description

Position Overview

Apex Serenity Medical Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. This full-time position is focused on providing exceptional mental health support to patients within our Hospice Center. The role is ideal for a social worker with a deep commitment to serving diverse and under-resourced populations, particularly those experiencing severe emotional disorders or major mental illnesses.

About the Organization

Apex Serenity Medical Clinic is committed to promoting healthier communities by delivering quality, safe, and patient-centered healthcare services. We operate primarily within medically underserved areas where we aim to deliver care regardless of an individual's ability to pay. Our mission drives our approach to healthcare, and we aspire to be a model for other health centers by effectively blending global strategies with local actions.

Work Environment

This remote position allows for flexibility while maintaining direct engagement with clients. Our Hospice Center provides a supportive and collaborative team environment conducive to professional growth. As part of a multidisciplinary team, you will engage not only with clients but also work alongside physicians, nurses, and allied health professionals to ensure seamless care transitions and holistic treatment.

Key Responsibilities
  • Work directly as a clinician with clients, conducting evaluations and providing therapy both remotely and in the community.
  • Establish and maintain positive rapport and trust with clients while delivering respectful, culturally competent services.
  • Collaborate with team members to enhance patient care strategies and advance common goals through effective communication.
  • Conduct brief, evidence-based screenings and utilize assessment tools to develop integrated care plans tailored to client needs.
  • Implement prevention, treatment, and recovery services based on individual client assessments and treatment goals.
  • Crisis intervention as needed, utilizing de-escalation techniques and resources available within the community.
  • Manage a caseload that includes individual, group, and family therapy sessions, ensuring that treatment focuses on presenting problems and client-requested outcomes.
  • Monitor client progress while ensuring compliance with admission, continued stay, and discharge criteria.
  • Complete reports accurately and submit data within required timelines to support clinic operations and client outcomes.
  • Participate in on-call coverage as assigned and engage in committee work aimed at improving agency service delivery and quality.
Patient Population / Client Focus

The primary client population includes individuals experiencing major mental illness and severe emotional disorders within the hospice care setting. This position requires an understanding of the unique challenges faced by clients in end-of-life scenarios, necessitating sensitivity, compassion, and a strong foundation in grief counseling and palliative care. Your work will directly impact the quality of life and emotional well-being of clients and their families during critical moments.

Qualifications
  • A Bachelor's degree in Social Work from an accredited institution.
  • Current licensure as a Clinical Social Worker (LCSW) in the state of Kansas.
  • A minimum of 2 years of direct experience in a clinical or social service setting, with a focus on mental health or hospice care preferred.
  • Demonstrated ability to work effectively with diverse populations and adhere to ethical standards and confidentiality requirements.
  • Strong organizational skills, attention to detail, and capability to manage time effectively within a remote work setting.
Preferred Experience
  • Experience in hospice or palliative care environments.
  • Skills in crisis intervention and conflict resolution.
  • Familiarity with integrated care models and interdisciplinary teamwork.
  • Proficiency in electronic health records (EHR) systems and data management.
Compensation and Benefits

The expected salary range for this position is USD 70,000 to 85,000 per year, commensurate with experience. Apex Serenity Medical Clinic offers a comprehensive benefits package that includes health insurance, retirement contributions, paid time off, and ongoing professional development opportunities to enhance your career.

Career Growth Opportunities

As a member of the Apex Serenity team, you will have access to continuous training and development programs, allowing you to keep pace with best practices in clinical social work and advance your skills. There are also opportunities to lead initiatives aimed at improving client care and to participate in research projects that can bolster your professional profile.

Why Join Our Team

Joining Apex Serenity Medical Clinic means becoming part of a mission-driven organization that prioritizes patient-centered care and community health. You will work in a supportive environment that values teamwork and innovative solutions to healthcare challenges. If you are passionate about making a difference in the lives of others and thrive in a collaborative setting, you will find a rewarding career with us.
