• Conduct thorough assessments and ongoing evaluations to identify client needs and tailor individualized treatment plans.
• Facilitate open communication with supervisors, direct care staff, and families, ensuring that all stakeholders are informed about client progress.
• Organize and conduct home visits to gather insights through observations, chart reviews, and staff interviews, enhancing understanding of client contexts.
• Document and track client progress accurately to maintain compliance with organizational standards and to contribute to quality assurance initiatives.
• Participate actively in Interdisciplinary Team Meetings to share insights and collaborate on client care strategies.
• Prepare monthly and quarterly reports that detail client progress and program effectiveness, serving as a tool for improvement and funding initiatives.
• Coordinate training sessions for team members, ensuring thorough understanding of program implementation and documentation standards.
• Manage confidential client records meticulously, making sure all information is securely accessible to authorized personnel.
• Act as a liaison between clients and the mental health community, working to enhance public awareness and access to support services.
• Contribute to the development of training materials aimed at improving individual and program outcomes.
