Change Makers LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity, serving our hospice center in Las Vegas, Nevada. This full-time position is remote, allowing for flexibility while actively contributing to our hospice center’s efforts from the comfort of your own home. Despite the remote nature of this role, you will be part of a closely-knit team committed to supporting each other through regular virtual meetings, consultation sessions, and ongoing training dedicated to enhancing the quality of care provided to our clients.

• Conduct thorough assessments of clients' social, emotional, and psychological needs to develop personalized care plans that cater to their unique circumstances.
• Provide supportive counseling to individuals and families confronting various challenges related to the hospice care process, utilizing proven therapeutic interventions.
• Facilitate both individual and group therapy sessions employing evidence-based practices, fostering an environment of healing and empowerment for clients.
• Develop and execute service plans that align with clients' objectives, ensuring that their preferences and values are at the forefront of care delivery.
• Advocate for clients’ rights and access to critical resources, engaging in efforts to promote social justice and equity in end-of-life care.
• Collaborate with multidisciplinary teams including medical professionals, community organizations, and family members to optimize service delivery and enhance client outcomes.
• Engage in program development, evaluation, and grant reporting, utilizing data to ensure accountability and drive improvement in service offerings.
• Participate actively in team meetings, training workshops, and community outreach initiatives, ensuring continuous professional development and community connection.

In this role, you will primarily work with hospice clients and their families, who are navigating the complexities associated with terminal illness and end-of-life care. The focus is on delivering compassionate, holistic support that addresses not only the psychological and emotional needs of clients but also engages their families in the care process, fostering a supportive environment during a challenging time.

• Master's degree in Social Work from an accredited program.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Proficiency in trauma-informed care approaches, social justice principles, and effective advocacy techniques.
• Excellent communication skills, with a particular emphasis on empathy and active listening.
• Strong organizational abilities to manage client caseloads and documentation efficiently.

• Prior experience working within a nonprofit organization, especially in a hospice or palliative care setting, is highly desirable.
• Familiarity with community resources in the Las Vegas area that can support client needs.
• A background in program evaluation or grant writing is an asset.
