• Conduct comprehensive assessments to determine the mental health needs of patients, utilizing evidence-based practices to inform your clinical approach.
• Maintain clear and open communication with supervisors, family members, and healthcare team members about patient progress and treatment plans.
• Draft and manage a detailed schedule of appointments, home visits, and other necessary observations to ensure ongoing patient support and program effectiveness.
• Plan and implement home visits, conducting thorough observations and data analyses to maintain high standards of care.
• Regularly evaluate the execution of program plans, providing feedback and adjustments to ensure successful patient outcomes.
• Participate actively in Interdisciplinary Team Meetings to discuss treatment strategies and contribute to collaborative care efforts.
• Document monthly and quarterly progress notes that reflect patient advancements and areas of focus.
• Organize and facilitate training sessions for team members to ensure proficiency in program implementation and documentation.
• Provide ongoing follow-up through interviews and observations to ensure that patient needs continue to be met.
• Act as a liaison with the public and the broader mental health community, promoting holistic development and community support initiatives.

This role will involve working with a diverse clientele, including both adults and youth across various backgrounds. Your expertise will be essential in addressing a wide range of mental health issues, including anxiety, depression, trauma, and substance misuse. Your ability to connect with clients will enhance therapeutic rapport and foster lasting positive changes in their mental health journey.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• An active and unrestricted license as a Professional Counselor in Colorado is necessary to perform the role effectively.
• A minimum of 2 years of counseling experience, particularly within a clinical setting, is required for consideration.
• Demonstrated specialization in areas such as family therapy, couples therapy, substance misuse, or trauma will be viewed favorably.
• A robust understanding of legal and ethical standards governing counseling practice is essential.
• Exceptional interpersonal and communication skills, allowing you to build rapport with a wide range of clients and colleagues.

• The annual salary for this position ranges from $80,000 to $105,000, depending on experience and caseload.
