• Conduct thorough evaluations and assessments to inform individualized service plans.
• Engage in the development and implementation of Individualized Family Service Plans (IFSP), actively involving family members.
• Regularly monitor client outcomes and adjust plans accordingly, utilizing data-driven methods to promote optimal development.
• Deliver support and coaching to families, helping them navigate strategies that enhance their child’s learning and development.
• Ensure compliance with all Federal and State regulations pertinent to social work and service delivery.
• Establish positive rapport with children and families to foster a trustful and effective therapeutic environment.
• Adapt methodologies to support different learning styles, providing tailored interventions related to each child’s unique needs.
• Utilize the coaching model and implement Natural Learning Environment Practices to achieve developmental milestones for children.

This role focuses on young children, particularly those experiencing developmental delays or disabilities. You will engage with children from birth to age three, assisting them to learn critical skills in physical, cognitive, emotional, and communicative domains. Collaborating with families, you will be a crucial partner in their journey, providing both assessments and hands-on interventions that foster holistic development.

• Master's Degree or Doctorate in Social Work.
• Licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
• A valid driver's license is required for potential in-home visits.
• Ability to adapt therapeutic approaches to cater to various learning styles effectively.
• Demonstrated commitment to applying Natural Learning Environment Practices in service delivery.
