• Conduct thorough psychosocial assessments using interviews and medical record reviews to understand the emotional, social, and environmental factors impacting patients' recoveries.
• Provide empathetic counseling and support to patients and their families, addressing emotional adjustment issues, grief, and overall well-being.
• Work collaboratively with the rehabilitation team to develop and implement individualized treatment plans that align with the specific psychosocial needs of each patient.
• Assist patients in navigating community resources, helping them access essential services like support groups, vocational rehabilitation, and housing assistance.
• Support and educate families on available community resources and post-rehabilitation services to ensure seamless transitions.
• Participate actively in discharge planning to facilitate safe transitions for patients back to their homes or other care settings.

Your contributions will directly impact a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. By addressing both the emotional and practical needs of patients, you play a pivotal role in enhancing their autonomy and successful reentry into the community.

• Master's degree in Social Work from an accredited program.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A strong understanding of adjustment issues, grief, coping mechanisms, and their impact on patient recovery experiences.
• Excellent communication and interpersonal skills to build trusting relationships with patients and colleagues alike.

• Prior experience in a rehabilitation or medical setting is highly desirable.
