Job Description:
The Atlanta Regional Commission (ARC) is the regional planning and intergovernmental coordination agency that focuses on issues critical to the region's success, including growth and development, transportation, water resources, services for older adults and workforce solutions. ARC is dedicated to unifying the region's collective resources to prepare the metropolitan area for a prosperous future. This is done through professional planning initiatives, the provision of objective information, and the involvement of the community in collaborative partnerships.
The Registered Nurse is responsible for providing clinical support and consultation to intake staff, including completing Level of Care (LOC) reviews for individuals being screened for and referred to the Elderly and Disabled Medicaid Waiver Program (EDWP) to ensure accurate assessment and referral of individuals seeking services. This position involves conducting telephone and face-to-face clinical reviews, identifying service needs of older adults, individuals with disabilities of all ages, caregivers, and members of the public seeking information about both public and private long-term care options and other available resources. The RN will determine level of care required, apply Intermediate Level of Care Criteria as defined in the CCSP Care Coordination manual, enter level of care screening data into required client management database(s), place clients who meet LOC on the CCSP waiting list, notify clients of referral outcomes including issuing denial letters, and provide additional resources to individuals as needed.
In addition to clinical responsibilities, this position functions within the framework of the Aging and Disability Resource Connection (ADRC), assisting individuals across the Atlanta region in accessing community-based support and services. This includes providing information and referral counseling and intake and screening for the Elderly and Disabled Waiver Program to determine service eligibility and needs.
This role is fully remote after the introductory period. There are occasionally times that this position requires coming into the office. The weekly hours for this position will vary based on the number of clients needing review, but we anticipate an average of 10 hours weekly.Individual must reside in Georgia.
Essential Duties and Responsibilities:
  • Provides Level of Care (LOC) reviews on individuals being screened/referred to EDWP.
  • Provides nursing consultation to intake staff on clinical issues.
  • Provides telephone or face-to-face consultation to determine program eligibility; assesses the needs of clients using a person-centered approach to determine resource options; and performs intake and screening to determine program eligibility.
  • Notifies clients of referral status.
  • Informs applicants of appeal rights.
  • Documents client interactions in client management system including assessment, identified needs, referrals and outcome.
  • Conducts re-screening of clients on waiting lists to certify continued need for services.
  • Conducts follow-up with clients to ensure resources are received, are appropriate, and individual outcomes are achieved.
  • Performs other duties assigned to support agency goals and objectives.

Minimum Qualifications:
  • Completion of a Registered Nurse program and two years of clinical nursing experience

  • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job, such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.

SPECIAL REQUIREMENT:
  • Selected candidate will be subject to a criminal background check.

LICENSING REQUIREMENT:
  • State of Georgia Registered Nurse License.
