Key Responsibilities

• Conduct comprehensive mental health evaluations and accurately render diagnoses to inform treatment strategies.
• Design and implement individualized treatment plans tailored to the unique needs of each patient and their specific situations.
• Provide weekly psychotherapy sessions to an established caseload with a focus on continuity of care.
• Engage with an interdisciplinary team of healthcare professionals to coordinate and enhance patient care strategies.
• Perform crisis intervention as required, employing appropriate techniques to support patients during critical moments.
• Comply with organizational policies and procedures, ensuring adherence to HIPAA and Centers for Medicare & Medicaid Services (CMS) regulations to protect patient privacy and rights.

Patient Population / Client Focus

The position primarily serves senior populations, addressing mental health needs prevalent in geriatric care, including adjustment disorders, dementia-related challenges, and general emotional support. Daily, you will foster relationships with patients who often face significant life transitions and emotional difficulties, requiring heightened empathy and a compassionate approach to therapy.

Qualifications

• Possession of a Master’s degree in Social Work (MSW) is mandated.
• Active licensure as a Clinical Social Worker (LCSW) is required.
• Strong communication skills, both verbal and written, to ensure clarity in patient interactions and documentation.
• Effective organizational skills to efficiently manage a caseload of clients.
• Compassionate demeanor with a commitment to understanding the diverse backgrounds and challenges faced by clients.

Preferred Experience

Compensation and Benefits
