Key Responsibilities

As a Licensed Professional Clinical Counselor, you will:

  • Assist primary care providers in identifying and managing mental health and psychosocial issues, contributing significantly to the primary care team.
  • Conduct comprehensive patient intakes, focusing on both diagnostic and functional evaluations to inform treatment recommendations.
  • Implement brief and focused therapeutic interventions for patients requiring mental health support, ensuring timely and effective care.
  • Advise primary care providers on which patients would benefit from continued care within the primary care setting versus referral to specialized mental health services.
  • Initiate timely follow-ups to assess patient conditions and adapt treatment plans as necessary.
  • Develop relapse prevention strategies for patients to maintain mental stability and overall well-being.
  • Monitor health service delivery related to behavioral care, coordinating with external providers while respecting patient confidentiality.
  • Document patient progress meticulously in treatment charts while keeping primary care providers informed of patient needs and treatment responses.
  • Encourage lifestyle changes and provide patients with self-management skills to actively participate in their treatment journey.
  • Consult with clinic management and team members on behavioral health services and program evaluations, contributing to process improvements.
  • Assist patients in adhering to medical treatments initiated by primary care providers, including providing strategies to cope with potential medication side effects.
  • Commit to ethical standards by adhering to the Code of Ethical Conduct for Clinical Professionals.
  • Undertake additional duties as assigned to support clinic functions and patient care.
Patient Population / Client Focus
