Spanish-Speaking Licensed Practical Nurse (LPN/LVN) - Nevada

Location: 100% Remote (Must reside in the United States)

About the Role

PharmD Live is seeking a compassionate, bilingual Licensed Practical Nurse (LPN/LVN) who is fluent in Spanish and English to join our growing clinical team. In this fully remote role, you will support patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs by providing ongoing education, care coordination, and clinical support.

Working collaboratively with physicians, pharmacists, nurse practitioners, and other healthcare professionals, you will help patients better manage chronic conditions, improve medication adherence, reduce unnecessary healthcare utilization, and achieve better health outcomes.

Key Responsibilities

  • Conduct comprehensive patient assessments to identify clinical, educational, and psychosocial needs.
  • Provide patient education in both Spanish and English regarding chronic disease management, medications, nutrition, preventive care, lifestyle modifications, and self-management strategies.
  • Develop and update individualized care plans in collaboration with the interdisciplinary care team.
  • Coordinate care across providers, specialists, pharmacies, and community resources to ensure continuity of care.
  • Monitor Remote Patient Monitoring (RPM) data, identify trends or clinical concerns, and escalate findings appropriately.
  • Enroll eligible patients into CCM and RPM programs while ensuring compliance with CMS program requirements.
  • Accurately document all patient encounters and care coordination activities within the electronic health record (EHR).
  • Build trusting relationships with patients through proactive outreach and ongoing engagement.
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.

Required Qualifications

  • Active, unrestricted Nevada Licensed Practical Nurse (LPN/LVN) license in good standing or willing to obtain a Nevada License.
  • Minimum of 2 years of clinical nursing experience, preferably in primary care, care coordination, chronic disease management, population health, or telehealth.
  • Fluent in both Spanish and English with the ability to communicate effectively with diverse patient populations.
  • Strong knowledge of chronic disease management and patient education principles.
  • Experience using Electronic Health Records (EHR) and healthcare technology platforms.
  • Excellent communication, organizational, and time-management skills.
  • Ability to work independently in a remote environment while managing multiple patient interactions.
  • Dedicated, HIPAA-compliant home office with reliable high-speed internet.

Preferred Qualifications
