Tapestry Management Providers (TMPs) play a crucial role in delivering comprehensive, in-person clinical support to healthcare facilities. They ensure proactive patient care through the review and discussion of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) & daily risk reports with facility healthcare professionals, including physicians, nurse practitioners, and social workers.
This is a full-time, hybrid position that combines remote work with weekly travel to assigned skilled nursing facilities in the North Columbus metro area. Providers are expected to travel within the assigned territory approximately three days with the remaining two days worked remotely.
Key ResponsibilitiesThe anticipated annual salary for this role is $125K - $135K with benefits including 401k, PTO, and CME. We also reimburse for mileage since travel is involved. This hybrid position follows a location-based compensation structure. The posted salary range represents the potential pay range. Actual compensation will be determined based on the candidate’s primary work location, experience, qualifications, and internal equity considerations, in accordance with applicable pay transparency laws.
