This role is entirely remote, allowing for flexibility in work hours while maintaining the same high level of support and care expected of our counselors. You will interact with patients primarily through teletherapy, making it essential to be proficient in electronic medical record (EMR) software and virtual communication platforms. • Conduct comprehensive assessments to identify the mental health needs of our patients and develop tailored treatment plans.
• Facilitate open communication among patients, family members, guardians, and interdisciplinary care teams regarding progress and adjustments in therapeutic approaches.
• Provide a structured schedule for clinical appointments, including follow-up home visits or observations as needed.
• Coordinate in-home observations and review charts to ensure continuous alignment with patient needs and treatment goals.
• Participate actively in interdisciplinary team meetings, offering insights and updates on individual patient cases.
• Maintain meticulous monthly and quarterly progress notes to track patient outcomes and program effectiveness.
• Organize training sessions for team members to ensure competency in executing the program and documenting accurately.
• Serve as a liaison with community resources and mental health agencies to enhance patient support services.
• Continuous follow-up with patients through interviews and additional support to facilitate ongoing engagement in their therapeutic journey.
• Contribute to the development of procedures and training materials aimed at improving program delivery and individual outcomes.

Your role will involve working with a diverse range of clients, including both adults and youth facing various mental health challenges. You will employ therapeutic models that cater to issues such as trauma, emotional distress, substance misuse, and relationship conflicts. Your expertise will be crucial in helping these individuals navigate their unique circumstances and achieve better emotional health.

• A Bachelor’s degree in Counseling, Psychology, or a closely related field is required.
• An active and unrestricted Professional Counselor (LPC) license in Arizona is essential.
• At least two years of counseling experience in a clinical setting is a must.
• Demonstrable skills in interpersonal communication with an ability to establish rapport with various clientele.
• Profound knowledge of legal and ethical standards relevant to clinical practice.
• Capacity to collaborate effectively within a healthcare team.
• Proficient in using EMR software and related technologies for documentation and communication purposes.

• Specialization in fields such as family therapy, couples therapy, substance use treatment, or trauma-informed care is highly desirable.
• Experience in developing and implementing innovative therapeutic approaches to enhance patient engagement and outcomes.
