• Deliver supportive counseling and mental health education to patients and their families, helping them navigate complex emotional and psychological challenges.
• Conduct thorough assessments and develop individualized treatment plans aimed at addressing the mental health needs of each client.
• Maintain an up-to-date knowledge of government and private payer networks, ensuring smooth transitions in care and maximizing patient access to necessary services.
• Coordinate care effectively by collaborating with an interdisciplinary team of healthcare professionals, ensuring that the patient’s comprehensive needs are being met in a timely manner.
• Advocate for patient rights, providing essential support and guidance to help families access protective services when necessary.
• Identify and report any signs of vulnerability or suspected abuse in compliance with legal and ethical standards.
• Monitor patient progress through electronic medical records, documenting interventions and updates to treatment plans as required.
• Create and facilitate transition conferences with patients and families to discuss care plans, goals, and support resources.
• Utilize advanced technology and screening tools to identify at-risk patients, implementing strategies to reduce the potential for readmission.
• Fulfill additional responsibilities as assigned by management that contribute to the overall effectiveness of our services.

Our client population includes individuals experiencing acute, long-term, or terminal mental health conditions. • A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution.
• Possession of an independent license in the state of residence as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Counselor (LPC).
• Strong communication skills, particularly in utilizing electronic mail and electronic medical records for effective patient interaction and record-keeping.
