• Provide empathetic support and information to patients and their families regarding transition plans, helping them understand available resources.
• Stay informed about local and national government and private payer networks to facilitate appropriate patient transitions.
• Collaborate closely with community organizations and healthcare providers to assure coordinated, comprehensive care for patients.
• Advocate for patients’ rights, ensuring they have access to protective services and are treated ethically.
• Conduct evaluations and assessments, demonstrating awareness of patient rights and decision-making guidelines.
• Report any concerns regarding vulnerability or suspected abuse in accordance with legal requirements.
• Participate in preventive care processes aimed at minimizing hospital readmissions for at-risk patients.
• Lead and organize transition planning meetings with patients and families to ensure clarity in ongoing care plans.
• Utilize advanced tools and technology to proactively identify patients who may be at risk of readmission, ensuring early intervention.
• Ensure effective clinical handoffs for patients identified as at risk, which may involve coordinating referrals as required.
• Document patient progress toward goals and maintain detailed records in our electronic medical record system.
• Foster constructive communication and collaboration with a diverse interdisciplinary team to advance patient care plans efficiently.
• Perform other duties as assigned to enhance the overall health care delivery system.

The successful therapist will work primarily with a diverse set of individuals and families experiencing a range of psychological challenges. Our client demographics include those grappling with mental health disorders tied to acute and chronic health issues. The role entails providing tailored interventions that address not only the psychological aspects but also the socioeconomic factors affecting mental wellness.

• A Master’s or Doctorate degree in Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• Possession of an independent license in the state of residence, such as LCSW, LMFT, or LPC.
• Strong organizational skills with the ability to manage a diverse caseload effectively while interfacing with various healthcare providers and support staff.
• Familiarity with electronic medical records for documentation and management of patient statistics.
• Excellent communication skills, particularly in electronic correspondence.
