Key Responsibilities
  • Conduct thorough and ongoing assessments to identify and address the needs of each patient, utilizing evidence-based practices.
  • Facilitate communication with supervisors, family members, and other involved parties regarding client progress, ensuring a holistic approach to care.
  • Develop and submit a tentative schedule of appointments, home visits, and required observations to supervisors, thereby ensuring continuity of care.
  • Coordinate and conduct home visits, which include in-home observations, detailed chart reviews, and collaborative staff interviews for comprehensive evaluations.
  • Implement program plans effectively, constantly evaluating their success and adjusting strategies as needed.
  • Participate in Interdisciplinary Team Meetings and related discussions, contributing insights that help shape treatment approaches.
  • Prepare and complete monthly and quarterly progress notes documenting patient developments and treatment outcomes.
  • Organize and lead training sessions, certifying staff and ensuring competency in program plan implementation and documentation practices.
  • Provide ongoing support through follow-up interviews, home visits, and observational assessments tailored to individual client needs.
  • Maintain organized and accessible records to ensure that critical information is readily available to authorized stakeholders.
  • Act as a liaison with the broader public and mental health community, supporting program development and enhancing service delivery.
  • Contribute to the development of training materials aimed at improving therapeutic programs for diverse individuals.
Patient Population / Client Focus

You will work with a wide range of populations including children, adolescents, and adults facing various mental health challenges. This includes individuals dealing with trauma, relationship issues, substance misuse, and other mental health disorders. Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
