Oaks Integrated Health Centre is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our dynamic team in Gresham, Oregon. This position is housed within our outpatient facility but operates primarily in a remote capacity, offering flexibility and convenience for our counselors. You will engage with clients in a supportive, technology-enhanced environment, employing electronic medical record (EMR) software and other digital communication tools to facilitate effective care. • Conduct comprehensive assessments to evaluate the specific needs of patients, leading to tailored therapeutic interventions.
• Maintain open communication with supervisors, direct care staff, families, and other stakeholders to discuss patient progress and treatment plans.
• Develop and provide a detailed schedule of appointments, home visits, and clinical observations to ensure consistent patient care delivery.
• Coordinate home visits to assess patients in their environments, conducting thorough chart reviews and gathering data for treatment planning.
• Implement and monitor the effectiveness of individualized program plans to ensure goals are achieved and adjustments are made as necessary.
• Actively participate in Interdisciplinary Team Meetings, contributing insights and updates to enhance collaborative patient care.
• Document and complete monthly and quarterly progress notes to track patient developments and inform ongoing treatment strategies.
• Organize and lead training sessions for staff to ensure proficiency in program execution and documentation.
• Provide ongoing follow-up with clients through interviews and home visits to address evolving individual needs.
• Maintain meticulous records to ensure information is readily accessible for collaborative efforts and compliance with regulations.
• Serve as a liaison within the mental health community to support the development of effective mental health initiatives.
• Assist in creating training materials and resources aimed at enhancing program effectiveness for our clients.

This role encompasses a diverse range of individuals, including both adults and children facing various mental health challenges. You will work closely with clients who may be dealing with family dynamics, trauma, or substance misuse, providing tailored therapy that aligns with their unique needs and goals.

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
• A minimum of two years of clinical counseling experience, demonstrating competence in delivering therapeutic interventions.
• Strong foundation in the ethical and legal standards governing mental health practice.
• Exceptional interpersonal and communication skills, with an ability to establish rapport with a varied client base.
• Demonstrated teamwork capability, collaborating effectively with diverse healthcare professionals.
• Proficiency in using EMR systems and related technology for efficient case management.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance use recovery is highly preferred.
• Experience working in a community mental health setting is advantageous, particularly in implementing culturally sensitive practices.

The salary for this position ranges from USD 90,000 to 100,000 annually, which is commensurate with your experience, credentials, and caseload.
