Key Responsibilities
  • Conduct ongoing comprehensive assessments tailored to the specific needs of each client, ensuring individualized care and support.
  • Maintain open lines of communication among supervisors, medical staff, families, and guardians to report on client progress and challenges.
  • Develop and provide a proactive schedule for client appointments, home visits, and comprehensive observations to monitor treatment efficacy.
  • Coordinate and execute judicious home visits that fulfill requirements for direct observations, client chart reviews, and staff interviews.
  • Guarantee that all program plans are effectively implemented and closely monitored for optimum client outcomes.
  • Engage actively in Interdisciplinary Team Meetings and other relevant gatherings to foster collaboration and share insights.
  • Document detailed monthly and quarterly progress notes that outline client developments and necessary adjustments to care plans.
  • Arrange and lead training sessions for staff to ensure competency in program plan implementations and documentation processes.
  • Support clients through ongoing interviews, home visits, and observations for continuous care and assessment of their needs.
  • Maintain accurate records to ensure transparency and accessibility to necessary information for clients, families, and healthcare partners.
  • Serve as a liaison between clients and the mental health community, facilitating support for developmental goals.
  • Contribute to the creation of procedures that enhance training materials and improve client programs.
Patient Population / Client Focus

In this role, you will have the opportunity to work with a diverse client population, including both adults and youth facing various mental health challenges. Your insights and expertise will play a crucial role in addressing issues related to family dynamics, couples counseling, substance misuse, and trauma recovery.

Qualifications
  • A Bachelor's Degree in Counseling, Psychology, or a related field is a minimum requirement.
  • Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Oregon is essential.
  • A minimum of two years of counseling experience is required, ideally within a clinical context.
