Job Duties:
As the Nurse Home Visitor you'll act as a case manager and implement standard protocols for families enrolled in the Nurse Family Partnership.
A typical schedule is part-time, 20 hours weekly, 8 am to 4:30 pm with the flexibility to work occasional evenings to accommodate client needs.
This role covers Northumberland County; you'll complete home visits each workday. It's a community-based position where you'll meet clients in their homes and other in-person community settings.
At least 2 years of RN OB and/or Pediatric experience strongly preferred.
Previous nursing experience in home visits, behavioral health, or public health is helpful.

Position Details:
You'll assess physical, emotional, social, and environmental needs of women and their families. Providing guidance and instructions to mothers and significant others regarding pregnancy, infants, and toddlers based on the standardized approach of the Nurse Family Partnership.
  • Provides overall coordination of care and home visitation for families enrolled in the Nurse Family Partnership.
  • Implements Telehealth per National service office guidelines and agency policy.
  • Assesses physical, emotional, social and environmental needs of women and their families as they relate to the Nurse Family Partnership domains.
  • Initiates a plan of visitation based on the standardized protocols of the Nurse Family Partnership.
  • Assists women and their families in establishing goals and outcomes.
  • Provides anticipatory guidance and instruction to mothers and significant others regarding pregnancy, infants and toddlers based on the standardized protocols of the Nurse Family Partnership.
  • Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals.
  • Evaluates the effectiveness of the plan of visitation and the outcome of each visit.
  • Completes and submits all appropriate documentation forms within 48 hours.
  • Provides for discharge planning.
  • Educates referral sources and the public about the Nurse Family Partnership.
  • Develops and maintains community relationships to support client referrals.
  • Completes all required Nurse Family Partnership education and orientation.
  • Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
  • Participates in review and analysis of reports and other reports available through Nurse Family Partnership for achievements and areas for improvement.
