You will collaborate virtually with physicians, nurses, and other healthcare professionals to deliver coordinated care. • Conduct comprehensive clinical assessments to formulate appropriate treatment plans tailored to each individual patient’s needs.
• Provide individual, family, and group therapy sessions, incorporating evidence-based therapeutic techniques to facilitate patient progress and emotional well-being.
• Collaborate closely with multidisciplinary teams, including medical staff and nursing professionals, to deliver holistic care and share insights regarding patient behaviors and needs.
• Offer crisis intervention services and emotional support, equipping patients and their families with coping strategies during times of distress.
• Maintain accurate and up-to-date documentation of patient interactions and progress, adhering to ethical and regulatory standards.
• Advocate for patients, ensuring they have access to necessary resources, support services, and education throughout their healthcare journey.
• Facilitate educational workshops for families on grief, coping strategies, and support options available.

This role focuses on patients within a hospice care framework who are facing terminal illnesses. You will work with a diverse population including elderly patients and their families, providing tailored mental health interventions that address the unique challenges of caring for individuals during significant life transitions. Your interactions will empower families by equipping them with tools and resources to navigate this emotionally complex experience.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• A valid California Licensed Clinical Social Worker (LCSW) license.
• Minimum of two years of relevant experience in a clinical setting, preferably within hospice or palliative care.
• Deep understanding of therapeutic techniques, especially in grief counseling and crisis intervention.

While not mandatory, experience working in a hospice or end-of-life care environment is highly desirable, as it equips you with insights into the emotional aspects of terminal illness. Familiarity with Electronic Health Record (EHR) systems and documentation practices in healthcare can enhance workflow efficiency and patient care management.

The salary for this position ranges from USD 90,000 to 100,000 per year, commensurate with experience and qualifications.
