• Conduct thorough mental health assessments that incorporate psychosocial evaluations, diagnostic interviews, and risk assessments to understand patient needs fully.
• Work with patients to create customized treatment plans that reflect their individual goals, cultural context, and specific scenarios regarding their care.
• Utilize evidence-based therapeutic methods such as CBT, DBT, and Solution-Focused Brief Therapy (SFBT) in individual, couples, family, and group therapy sessions.
• Deliver urgent crisis intervention services when necessary, ensuring seamless communication with other healthcare professionals to facilitate continuous care during acute distress.
• Keep precise and current clinical documentation, including progress notes and treatment plans, aligning with clinic policy and state regulations to maintain high standards of patient care.
• Educate patients and their families about mental health challenges, empowering them with strategies for managing their conditions effectively.
• Collaborate with a multidisciplinary team to ensure coordinated patient care, acting as a vital link between various health service providers.
• Regularly assess the effectiveness of therapeutic interventions and adjust treatment plans as needed to optimize patient outcomes.
• Stay informed about the latest research and best practices in mental health, integrating new knowledge into your clinical approach.
• Adhere strictly to ethical guidelines and maintain patient confidentiality in all interactions and documentation.

• A Master’s degree in Social Work (MSW) is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) to practice.

• Strong interpersonal and communication skills that enable you to build rapport with a diverse array of patients and families.

• Experience working in hospice, palliative care, or similar settings is highly desirable.
