• Conduct in-depth behavioral health psychosocial assessments for patients and their families to formulate individualized treatment plans tailored to their unique situations.
• Provide individual, group, and family therapy, utilizing evidence-based therapeutic modalities to support patients in coping with illness, grief, and transitions related to hospice care.
• Develop and manage a comprehensive case management system for assigned patients, ensuring ongoing support and follow-up to assess progress and adapt treatment as necessary.
• Collaborate with physicians, nurses, and other healthcare providers to create integrated care plans, facilitating seamless communication and coordinated interventions.
• Identify and mobilize community resources, such as support groups and financial assistance programs, to empower patients and their families, enhancing their quality of life.
• Respond effectively to crisis situations, providing immediate support and interventions, including assessments for potential harm and necessary referrals.
• Maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress, complying with healthcare regulations and confidentiality standards.

This role primarily serves patients in a hospice setting, many of whom are facing terminal illnesses and require compassionate support during a critical time of their lives. You will engage with diverse populations, ensuring access to appropriate resources and treatment methods that respect cultural, social, and individual differences.

• A Master’s Degree in Social Work from an accredited institution, with a focus on clinical practice.
• Active California BBS Licensed Clinical Social Worker (LCSW) certification is mandatory.
• A minimum of two years of experience delivering behavioral health services, specifically within a hospice or end-of-life care context.
• Proven experience working with Veterans and an understanding of their specific mental health and social service needs.
• Demonstrated skills in taking psychosocial histories, performing safety assessments, and executing case management evaluations.
• Strong therapeutic skills in both individual and group settings, with the ability to navigate family dynamics during challenging situations.
• Familiarity with local community resources available for patients and their families, facilitating referrals as necessary.

• Experience in palliative care or hospice settings is highly desirable.
• Knowledge of evidence-based treatment approaches and an understanding of ethical considerations in end-of-life care.
