ABOUT SENSORUM HEALTH

Our next-generation ambient, home-based sensor technology employs advanced analytics to detect the early onset of illness, alerting Sensorum's clinical team to take immediate action and deploy tailored, outpatient care to patients in-need.

ROLE
As a Part-Time (1099) Remote Nurse at Sensorum Health, you will leverage your clinical expertise, particularly in emergency care, to identify patients experiencing clinical decompensation. You will monitor and interpret patient data, conduct telephonic assessments, and manage care coordination to ensure timely interventions. This role is ideal for a highly skilled nurse who excels at remote clinical decision-making, patient triage, and proactive care management.
Hours: 12-16 hours per week (remote). There may be an opportunity to expand these hours as our patient panel grows.
RESPONSIBILITIES
Remote Patient Monitoring and Clinical Triage:

  • Review patient data from Sensorum's remote monitoring system to identify signs of clinical decompensation.
  • Assess patients for exacerbation of chronic conditions (e.g., CHF, COPD, diabetes, dehydration, pneumonia).
  • Conduct timely telephonic assessments with patients and/or caregivers to determine clinical status.
  • Triage and escalate cases requiring intervention, including coordinating transport to outpatient clinics, urgent care, or the ER as appropriate.
  • Provide clinical decision-making support and recommendations for next steps in care.
Care Management and Coordination:
  • Engage patients and families to provide education on managing chronic conditions and early warning signs.
  • Collaborate with Sensorum's partners and operations teams to ensure timely follow-up and care delivery.
  • Document all assessments, clinical findings, and care recommendations accurately in the system.
Patient-Centered Approach:
  • Act as a trusted clinical resource, ensuring compassionate and proactive patient communication.
  • Identify opportunities to improve patient outcomes through early detection and intervention.
  • Support patient empowerment and satisfaction by leveraging technology for engagement and patient education.
REQUIREMENTS
  • Registered Nurse (RN) with an active license.
  • Minimum of 3 years of clinical experience, with ER, acute care, or triage experience strongly preferred.
  • Strong critical-thinking and clinical decision-making skills to identify decompensation and coordinate interventions.
  • Ability to effectively communicate with patients, caregivers, and healthcare teams in a virtual setting.
  • Tech-savvy and comfortable working with remote patient monitoring dashboards.
  • Passion for improving healthcare delivery and alignment with Sensorum Health's mission.
  • Availability for 12-16 hours per week on a consistent remote schedule.
PREFERRED QUALIFICATIONS
  • Experience in telephonic assessments and remote patient monitoring (RPM).
  • Experience managing older adult or high-risk patient populations.
  • Background in chronic disease management and care coordination.
  • Prior experience working in telehealth or startup environments.
