• Provide clinical consultation to our multidisciplinary clinical staff regarding complex mental health issues, with a focus on geriatric patients and substance use disorders.
• Interpret and relay critical information related to patient histories and family dynamics, ensuring seamless communication among providers and staff.
• Coordinate and actively contribute to the planning and decision-making processes concerning individual patient care plans, particularly addressing unique home and social situations.
• Develop treatment plans tailored to the needs of patients, continuously reviewing and adjusting these plans based on provider feedback and patient progress.
• Complete required documentation, including progress notes and reports, in an efficient and timely manner following client sessions.
• Assist in mentoring and training newer team members, sharing best practices and fostering a collaborative clinical environment.
• Engage in ongoing professional development and participate in team meetings to contribute to improved clinical practices and patient outcomes.

The diverse patient population you will serve includes individuals facing a variety of mental health challenges, including but not limited to anxiety disorders, depression, PTSD, geriatric issues, and substance use disorders. Each patient presents unique needs and backgrounds, offering meaningful opportunities for personalized intervention and therapeutic support.

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.
• Possession of an independent clinical license such as LPCC, LCSW, or LMFT is mandatory.
• Demonstrated proficiency in completing clinical documentation and progress notes efficiently post-session.
• Strong interpersonal skills and the ability to effectively communicate with clients, families, and multidisciplinary team members.

• Experience working with geriatric populations and individuals with substance use disorders.
• Prior experience in outpatient settings, demonstrating the ability to handle a varied caseload effectively.
• Familiarity with electronic health record (EHR) systems for documentation and patient management.
