Key Responsibilities
  • Provide clinical consultation to our multidisciplinary clinical staff regarding complex mental health issues, with a focus on geriatric patients and substance use disorders.
  • Interpret and relay critical information related to patient histories and family dynamics, ensuring seamless communication among providers and staff.
  • Coordinate and actively contribute to the planning and decision-making processes concerning individual patient care plans, particularly addressing unique home and social situations.
  • Develop treatment plans tailored to the needs of patients, continuously reviewing and adjusting these plans based on provider feedback and patient progress.
  • Complete required documentation, including progress notes and reports, in an efficient and timely manner following client sessions.
  • Assist in mentoring and training newer team members, sharing best practices and fostering a collaborative clinical environment.
  • Engage in ongoing professional development and participate in team meetings to contribute to improved clinical practices and patient outcomes.
Patient Population / Client Focus

The diverse patient population you will serve includes individuals facing a variety of mental health challenges, including but not limited to anxiety disorders, depression, PTSD, geriatric issues, and substance use disorders. Qualifications
