• Conduct comprehensive mental health assessments that consider co-occurring medical conditions impacting patient well-being.
• Design and implement tailored, integrated treatment plans that address both mental health and physical health needs.
• Facilitate individual and group therapy sessions using evidence-based approaches, ensuring sessions are adapted to meet the specific needs and circumstances of each patient.
• Collaborate with medical providers to coordinate care efforts, ensuring effective communication and optimal treatment outcomes for patients.
• Provide immediate crisis intervention and support during mental health emergencies, empowering clients to manage crises effectively.
• Educate patients about the relationship between mental health and physical health, equipping them with strategies to manage their conditions independently.
• Maintain comprehensive, accurate documentation that adheres to legal and ethical standards while protecting patient confidentiality.
• Engage in regular team meetings and interdisciplinary planning sessions to enhance collaborative care efforts.
• Stay informed about current research, best practices, and advancements in integrated care methodologies.

Our client base includes individuals with complex mental health needs often compounded by co-occurring medical conditions. As a therapist in our outpatient setting, you will work with diverse populations, including adults experiencing anxiety, depression, trauma, and substance use disorders, requiring a compassionate yet professional approach to care.

• Master's or doctorate in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current licensure as an LCSW, LPCC, or LMFT in the state of California.
• Strong interpersonal and communication skills to facilitate effective interactions with patients and healthcare teams.
• Proven ability to maintain professional boundaries and adhere to ethical guidelines, ensuring confidentiality and support for all clients.
• Familiarity with electronic health record systems and telehealth applications to provide quality care.

• Experience working with clients that have co-occurring medical conditions and mental health issues.
