Key Responsibilities

• Conduct virtual individual and group therapy sessions, addressing the unique emotional and psychological needs of hospice patients and their families.
• Develop, implement, and refine personalized treatment plans tailored to each client's specific circumstances, taking into account their mental health needs and life transitions.
• Collaborate with interdisciplinary team members—including physicians, nurses, and caregivers—to coordinate comprehensive care and ensure continuity of support.
• Maintain meticulous and confidential client records in accordance with legal and ethical standards, utilizing electronic health record (EHR) systems to document progress and treatments.
• Facilitate support groups and workshops focused on coping strategies for families experiencing grief and loss, thereby fostering a community of support.
• Provide crisis intervention when necessary, employing effective de-escalation techniques and referral systems to assist clients in critical situations.

Patient Population / Client Focus

Your role as a licensed clinical social worker will primarily focus on supporting patients within the hospice setting. This includes helping individuals facing terminal illnesses and their families navigate complex emotional experiences. You will engage with a diverse population, requiring cultural sensitivity and an awareness of varying backgrounds and belief systems, to provide empathetic and effective support tailored to evolving patient needs.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.
• A minimum of two years of post-licensure experience in a clinical setting, with a preference for experience in hospice or palliative care.
• Proficient in conducting diagnostic assessments and developing individualized treatment plans that resonate with client goals.
• Exceptional interpersonal and communication skills, with an emphasis on building rapport across diverse populations.

Preferred Experience

Compensation and Benefits
