• Conduct thorough assessments of clients' social, emotional, and environmental challenges, ensuring a complete understanding of their needs.
• Develop and implement personalized service plans that include clearly defined goals, the methods of intervention, and ongoing progress monitoring.
• Provide individual and family counseling to assist clients navigating various life challenges, using evidence-based therapeutic techniques.
• Work collaboratively with clients to identify and connect them with relevant community resources, fostering a network of support.
• Perform crisis intervention as needed, including risk assessments and safety planning for individuals facing immediate threats or harm.
• Coordinate with local agencies and organizations, ensuring a comprehensive service delivery model and advocating fiercely for the needs of clients.
• Engage in community outreach activities with the intent to promote agency services and raise awareness about critical social issues.
• Participate actively in case conferences, interdepartmental meetings, and continuous professional training to enhance team collaboration and personal skill development.

As a Licensed Clinical Social Worker at Health Access Network, you will primarily support individuals and families grappling with significant life challenges. This includes those seeking help due to mental health issues, domestic crises, and socioeconomic hardships. • Master's degree in Social Work from an accredited institution, demonstrating strong foundational knowledge in social work principles.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Florida, ensuring compliance with professional standards and practices.
• Strong theoretical and practical understanding of topics related to social and economic justice, community resource navigation, and trauma-informed care.
• Excellent interpersonal, communication, and organizational skills to effectively engage with clients and collaborate with community partners.

• Prior experience in a social service agency or similar environment where case management and client advocacy were key responsibilities.
• Demonstrated ability to navigate diverse client needs and tailor interventions to meet specific circumstances effectively.
• Familiarity with evidence-based practices and their application in clinical settings, particularly those relevant to crisis intervention and mental health support.
