Key Responsibilities

  • Conduct thorough assessments of patients' mental health needs to create tailored treatment plans that promote effective therapeutic interventions.
  • Facilitate consistent communication regarding patient progress with supervisors, direct care team members, and family members, ensuring clarity and continuity in care.
  • Develop and manage a tentative schedule of appointments, home visits, and progress observations, documenting significant advancements to monitor overall therapeutic outcomes.
  • Coordinate and conduct in-home visits that adhere to specific therapeutic protocols, focusing on individual needs through observations, chart reviews, and data analysis.
  • Implement and monitor program plans, ensuring that each therapeutic approach is executed effectively to support patient progress and well-being.
  • Participate actively in Interdisciplinary Team Meetings and contribute to collaborative discussions that inform case management and patient care strategies.
  • Prepare and file monthly and quarterly progress notes that capture patients' therapeutic journeys and maintain comprehensive documentation.
  • Organize training sessions to enhance team members' competencies regarding program plan implementations and documentation standards.
  • Carry out follow-up interviews and additional assessments as needed, reinforcing support for individuals while tracking their progress toward goals.
  • Maintain organized records to guarantee that vital information is easily accessible for authorized personnel and stakeholders.
  • Act as a liaison between the clinic and the broader mental health community, contributing to public awareness and supporting development goals within the field.
  • Assist in the creation of training materials and procedures aimed at improving service delivery for clients.
