Job description

We're looking for a Licensed Practical Nurse (LPN) focused on remote patient monitoring and hospital avoidance. This role requires an organized professional to monitor patient data, conduct timely follow-ups for concerning symptoms, coordinate care, and actively engage patients to ensure adherence to their care plans and device usage.
Job Responsibilities:
  • Proactively manage patient health by monitoring remote patient monitoring (RPM) dashboards and responding to vital sign alerts (e.g., BP, glucose, weight)
  • Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage
  • Coordinate timely follow-up care with Nurse Practitioners (NPs), Primary Care Providers (PCPs), or patient caregivers
  • Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems
  • Ensure patient compliance and smooth operation by troubleshooting remote monitoring device issues
  • Act as the primary clinical point of contact for an assigned panel of remote care patients
  • Assist with patient enrollment and onboarding activities as needed to support growth.
  • Collaborate daily with the clinical team (NPs), intake staff, and support personnel
Requirements
  • Must possess an active LPN license in Virginia
  • Demonstrated strong clinical judgment combined with excellent verbal communication and phone skills
  • Highly organized, proficient with technology, and adept at managing simultaneous digital platforms
  • Prior experience in Remote Patient Monitoring (RPM), telehealth, or complex care coordination is highly desirable
  • A deep commitment to preventive care and reducing hospital readmissions/ER utilization.
  • Ability to thrive in a structured, fast-paced in-office team environment
Minimum Technical and Work Environment Requirements:
  • Internet Connection:
    • Primary internet connection with a minimum speed of 15 Mbps.
    • Backup internet connection with at least 10 Mbps.
    • Backup connection must be capable of supporting work during a power outage.
  • Primary Device:
    • Desktop or laptop equipped with at least:
      • Intel Core i5 (8th generation or newer), Intel Core i3 (10th generation or newer), AMD Ryzen 5, or an equivalent processor.
      • A minimum of 8 GB RAM.
