As a Care Navigator, you will be an invaluable member of our team, ready and eager to make an impact internally and to patients we serve. You will support patient success by
identifying the needs and challenges patients have and help to assist in implementing interventions to guide new thinking and create behavior change.
We're looking for an entrepreneurial individual to best serve our clients and patients, utilizing methodologies to support your patients through their recovery while developing trusting relationships. The ideal candidate is process-focused and eager to make an impact on lives to support musculoskeletal health.
You must be equal parts self-starter and team player. We are seeking a passionate candidate with expertise in engaging and supporting users to achieve their health goals, who is interested in taking on an impactful role at a young and quickly growing startup in the healthcare industry.
Responsibilities
  • You will provide remote monitoring services to help support patients as they progress through their home-exercise therapy program
  • Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform
  • Provide ongoing feedback to the evaluating clinician on patient progress outside of the clinic
  • Maintains necessary records and notes while providing remote monitoring
  • Assist with improving processes and functionality of the platform
  • Organize and manage a caseload while staying efficient

Requirements
  • As a PT, you are naturally empathetic, friendly, and love what you do!
  • 2+ years of experience in musculoskeletal health is a plus
  • Graduated from an accredited physical therapy program โ€ข Active PT license in a compact state ( https://www.ptcompact.org/ )
  • Tech Savvy - thrive in a multi-platform environment, comfortably switching between systems (EHR, messaging, scheduling) without missing a beat
  • Experience in behavioral modification, Health Coach Certification is a plus
  • Excellent communication skills, interpersonal, and presentation skills, and an ability to collect and present meaningful data to drive decisions
  • Strong relationship-building and listening skills
  • Demonstrated patient focus; a genuine passion for the user and their experience
  • Ability to multi-task and manage various types of users
  • Ability to work independently with limited supervision
  • The flexibility and agility to work cross-functionally in a fast-paced environment
  • The ability to influence change and bring enthusiasm and excitement to the workplace
  • Independent and proactive work style; quick, flexible thinking; and an open, inquisitive approach
  • Eager to make an impact at a young company addressing an important problem

Hourly Salary Range: $30.40 - $38.00 USD + up to a 12% ICP
