Key Responsibilities

• Conduct **comprehensive psychosocial assessments** through patient interviews, observations, and analysis of medical records to evaluate emotional, social, and environmental factors influencing recovery.
• Provide **individual and family counseling**, offering support to address adjustment issues, emotional well-being, and coping strategies.
• Collaborate closely with the rehabilitation team to create **tailored treatment plans**, ensuring that the psychosocial needs of each patient are prioritized.
• Assist patients in accessing vital **community resources**, including support groups, vocational rehabilitation, and housing assistance, empowering them to regain independence.
• Participate actively in **discharge planning**, ensuring patients transition smoothly back into their homes or other care settings by providing necessary education and resources.
• Maintain detailed documentation and progress notes in compliance with state regulations and clinic protocols, ensuring quality care is continuously delivered.

Patient Population / Client Focus

Your primary focus will be on individuals recovering from a variety of conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. These patients often face significant emotional challenges alongside their physical rehabilitation. Your role will be pivotal in addressing these emotional challenges and enhancing their mental fortitude for a smoother recovery journey.

Qualifications

• A **Master's degree in Social Work** from an accredited program.
• An active and valid state license as a **Licensed Clinical Social Worker (LCSW)**.
