Job description

Position Overview

The Eastern Valley Health Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. This full-time position is instrumental in delivering quality mental health services to clients grappling with major mental health issues and severe emotional disorders, primarily within our hospice center. Our LCSW will work closely with clients, their families, and healthcare providers to enhance patient care and promote overall wellness.

About the Organization

Located in Kansas City, Kansas, the Eastern Valley Health Clinic is committed to fostering healthier communities by providing accessible, safe, and patient-centered healthcare services. We focus particularly on medically underserved areas, ensuring that our services are available to all, irrespective of their ability to pay. Our organization operates with a vision to serve as a model for other health centers, continuously guided by our mission to improve health outcomes and promote equity in healthcare access.

Work Environment

This position offers the flexibility of a remote working environment, designed to accommodate the unique needs of our healthcare professionals while ensuring they have the tools they need to succeed. Working from home allows our LCSWs to create a productive and comfortable setting to engage with clients through telehealth services, community outreach, and follow-up consultations. While our focus is on remote service delivery, regular team meetings and supervisory sessions take place to foster collaboration and professional development.

Key Responsibilities
  • Act as a primary clinician for assigned clients with complex mental health needs, conducting assessments and providing treatment in both community and virtual settings.
  • Develop comprehensive care plans that address the individual needs of clients, ensuring integration of health services and continuous monitoring of progress.
  • Conduct timely, evidence-based screenings and arrange for more detailed assessments as necessary to avoid unnecessary delays in care.
  • Facilitate individual, family, and group therapy sessions to help clients develop coping strategies and achieve their personal goals.
  • Collaborate with multidisciplinary teams to implement best practices for client care, emphasizing shared decision-making and patient empowerment.
  • Maintain meticulous client documentation and ensure compliance with ethical guidelines and organizational policies.
  • Participate in on-call rotations to provide crisis intervention and support during after-hours as required.
  • Engage in ongoing professional development activities to enhance skills relevant to clinical practice and client needs.
  • Monitor treatment effectiveness through regular follow-up and adjustments to care plans as needed.
  • Contribute to quality improvement initiatives aimed at enhancing service delivery and client outcomes.
Patient Population / Client Focus

Our LCSW will primarily work with clients experiencing severe emotional challenges and mental illnesses, particularly within a hospice care context. This involves a sensitive approach to end-of-life issues and providing emotional support to clients and their families. Our ideal candidate understands the unique challenges faced by these individuals and can provide compassionate and skilled counseling aimed at improving coping mechanisms and overall quality of life.

Qualifications
  • A valid and current Licensed Clinical Social Worker (LCSW) license in the state of Kansas.
  • A Bachelorโ€™s Degree in Social Work or a related field; relevant advanced degrees are a plus.
  • Minimum of two years of relevant clinical experience in a mental health setting.
  • Strong commitment to serving diverse populations and understanding the social determinants of health.
  • Proficient in utilizing telehealth technology to conduct remote assessments and therapy sessions.
  • Exemplary communication skills with the ability to build trust and rapport with clients and colleagues.
Preferred Experience
  • Experience in hospice or palliative care settings.
  • Familiarity with integrated care models and interdisciplinary team approaches.
  • Experience with crisis intervention and managing high-acuity cases.
  • Background in community outreach and engagement strategies.
Compensation and Benefits

In recognition of the importance of this role, Eastern Valley Health Clinic offers an annual salary range of USD 70,000 to 85,000, commensurate with experience and qualifications. Additional benefits include health, dental, and vision insurance, a retirement savings plan with employer contributions, paid time off, and professional development funding.

Eastern Valley Health Clinic actively promotes from within and offers training programs to enhance clinical skills, leadership capabilities, and specialty training in areas of personal interest.
