• Conduct thorough psychosocial assessments to identify patients' behavioral health needs, using established assessment tools and methodologies.
• Develop, implement, and evaluate individualized treatment plans in collaboration with healthcare team members, tailoring interventions to address unique patient circumstances.
• Utilize your knowledge of cultural competency to provide age-appropriate care and treatment plans across diverse life stages—from children to the elderly.
• Maintain familiarity with medical diagnoses and terminology, facilitating effective communication regarding patients' psychosocial and behavioral health needs.
• Implement best practices in patient safety to minimize risks and enhance the quality of care provided during each interaction.
• Be a resource for the healthcare team on psycho-social issues, guiding other professionals regarding community resources and support systems available to patients and families.

Your role will focus on patients receiving hospice care, which requires specialized knowledge and sensitivity to the emotional and psychological challenges they face. • A Master's degree in Social Work (MSW) or a related field is required, with a focus on obtaining and maintaining active licensure as a Licensed Independent Clinical Social Worker (LICSW).
• A minimum of 2 years of experience in social services, specifically within psychiatric or mental health settings, is essential for this position.
• Current certification in safe management of disruptive/assaultive behavior adds value to your candidacy.
