Berks Community Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This role is critical in supporting patients and their families through the complexities of hospice care, ensuring emotional and psychological well-being at a pivotal time in their lives. The LCSW will earn an annual salary within the range of USD 90,000 to 100,000.

This position is fully remote, providing flexibility and work-life balance while supporting patients from home. Even though the LCSW is not physically at the hospice center, you will work closely with a multidisciplinary team that includes behavioral health providers, medical professionals, and community resources, fostering collaboration through technology and regular virtual interactions. All patient documentation will be managed through our electronic medical records (EMR) system.

• Establish rapport with patients and their families promptly, ensuring effective communication by employing active listening and non-judgmental interactions.
• Collaborate seamlessly within a professional multidisciplinary team, addressing the holistic needs of patients in hospice care.
• Conduct brief, evidence-based screenings for mental health conditions, including depression, anxiety, and substance use disorders, ensuring subsequent assessments are arranged when necessary.
• Manage a caseload of patients, providing timely follow-up interventions to enhance quality of care and continuity of support.
• Utilize the electronic medical record system to assure meticulous documentation of patient encounters within 48 hours, facilitating integrated care.
• Develop and implement comprehensive care plans that integrate various service needs and foster continuous communication with all stakeholders involved in each patient's care.
• Safeguard patient privacy and confidentiality in all communications and documentation practices.
• Apply principles of population-based care to address chronic conditions and promote preventive measures within the hospice setting.
• Engage actively in clinical supervision, case conferences, and departmental meetings to enhance quality improvement initiatives.
• Refer patients requiring psychiatric evaluations to BCHC’s consulting psychiatrist, ensuring comprehensive mental health services.
• Proactively return phone calls from patients and their families to address concerns and facilitate appointments.
• Assist patients in accessing community resources—including educational, spiritual, and vocational support—that align with their care goals.
• Perform other duties as assigned to support the hospice team and patient outcomes.

You will focus primarily on patients receiving hospice care, including individuals facing terminal illnesses and their families. This role emphasizes cultural competence and sensitivity, providing tailored support to underserved populations who may experience added challenges. You will utilize a trauma-informed approach to engage with individuals who may have diverse and complex backgrounds.

• A Master's degree in a behavioral health field is essential.
• Licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Current CPR or BLS certification is required.
• A minimum of 3 to 5 years of experience in a behavioral health setting is expected, particularly in contexts involving hospice or palliative care.
• Proficiency with electronic medical records and related technology is essential.
• Strong interpersonal skills to build relationships and foster trust with patients and colleagues.
• Attention to detail, problem-solving ability, and effective communication skills are crucial for success in this role.

• Bilingual proficiency in English and Spanish is preferred to enhance patient communication and accessibility.
• Experience working in environments serving underserved communities and familiarity with cultural competence principles.
• Knowledge of trauma-informed care and social justice frameworks is advantageous.
