• Evaluate the emotional and mental health needs of patients and provide tailored counseling interventions to develop effective transition plans.
• Utilize telehealth platforms to conduct virtual sessions, ensuring patient confidentiality and ethical standards are upheld.
• Maintain comprehensive knowledge of local and national payer systems to facilitate appropriate referrals and resource linkages for patients.
• Advocate for patients’ rights, ensuring access to necessary mental health resources and additional supportive services.
• Participate in interdisciplinary team meetings to discuss patient needs and strategize coordinated care approaches.
• Document all patient interactions and treatment progress in our electronic medical records system, mitigating risks related to information accuracy and patient safety.
• Participate in care transition conferences to engage with patients and their families, ensuring seamless continuity of care.
• Identify patients at risk for readmission and implement preventive measures through timely interventions.
• Provide educational resources and psychosocial support, assisting patients with social or financial issues that may affect their treatment outcomes.
• Engage in ongoing professional development and knowledge-sharing with peers to enhance therapeutic techniques and patient care.

• A Master’s or Doctorate degree from an accredited program in clinical psychology, counseling, or social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
• An independent license to practice in the state of Oregon is required (LCSW, LMFT, LPC).
• Evidence of proficiency with electronic medical records and telehealth technologies.
• Strong written and verbal communication skills suitable for interfacing with patients and interdisciplinary teams.
• Capability of effectively managing a sizeable caseload in a dynamic, remote setting.

• Prior experience in a telehealth setting or delivery of remote mental health services is highly desirable.
• A proven passion for improving mental health delivery and patient outcomes.
• Familiarity with treatment planning specific to a diverse array of diagnoses and conditions.
