• Conduct thorough and ongoing assessments to determine the immediate and ongoing needs of patients, ensuring that care plans are personalized and effective.
• Facilitate communication among all key stakeholders, including supervisors, direct care staff, family members, and guardians, to provide comprehensive updates on a patient’s progress.
• Develop and share a schedule for appointments, home visits, and observations, ensuring that all monitoring activities are documented and executed with precision.
• Plan and coordinate home visits to facilitate in-home evaluations, engage in chart reviews, conduct staff interviews, and analyze data for informed decision-making.
• Ensure the effective implementation of the program plan, tracking its execution and making necessary adjustments to meet identified objectives.
• Actively participate in Interdisciplinary Team Meetings, contributing insights and professional expertise to optimize patient care.
• Prepare detailed monthly and quarterly progress notes, documenting patient developments and outcomes.
• Organize and conduct training sessions designed to certify staff competency in program implementation and documentation practices.
• Conduct follow-up interviews, home visits, and observations to provide continued support for individuals, reinforcing their treatment process and recovery.
• Maintain meticulous records to ensure that information regarding patient care and program efficacy is readily accessible.
• Serve as a liaison with the public and mental health community, advocating for the development of program goals and furthering our collective mission.
• Assist in the creation of training materials and procedures to continuously improve our programs and ensure they meet the evolving needs of our clients.

Our LPCs work with a diverse clientele, including adults and youth from various backgrounds. We specialize in addressing concerns such as family dynamics, trauma recovery, substance misuse, and relational issues. By understanding our clients thoroughly, we are better equipped to deliver tailored interventions that align with their unique circumstances and aspirations.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential.
• Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Georgia is mandatory.
• A minimum of two years’ experience in a clinical counseling context is required, showcasing a deep understanding of therapeutic techniques and patient care.
• Specializations in family therapy, couples therapy, trauma-informed care, or substance misuse treatment are highly preferred.
• A strong understanding of the legal and ethical standards governing mental health practice is critical.
• Exceptional interpersonal and communication skills, enabling you to build rapport with diverse clientele effectively.
• Demonstrated ability to work collaboratively within a healthcare team is crucial for this role.
• Proficiency in using Electronic Medical Records (EMR) and relevant technologies will be required to efficiently manage patient information.
