• Collaborate with primary care providers to recognize, diagnose, and manage mental health and psychosocial concerns.
• Conduct thorough patient intakes, including diagnostic and functional evaluations, and provide actionable recommendations for treatment plans.
• Implement brief, focused therapeutic interventions for patients needing immediate mental health support.
• Maintain ongoing communication with primary care providers regarding patient progress and needs, ensuring timely adjustments to treatment plans.
• Develop and manage relapse prevention plans to assist patients in maintaining mental health stability.
• Monitor patients identified as 'at risk,' implementing preventive strategies to avert deterioration of their condition.
• Document patient progress and diagnostic information accurately in electronic health records, ensuring compliance with federal and organizational guidelines.
• Provide patients with self-management skills and educational resources to empower their active participation in their recovery journeys.
• Offer counseling and support to patients to help them adhere to medical treatments initiated by their primary care providers.
• Adhere to the Code of Ethical Conduct for Clinical Professionals and contribute to organizational initiatives regarding behavioral health.

The counselor will serve a diverse patient population, including children, adolescents, adults, and families facing a variety of mental health challenges.
