The Senior Support Specialist will carry out a variety of responsibilities designed to foster positive outcomes in the lives of our clients. Key duties include:

• Developing individualized care plans in collaboration with clients and their families to achieve specific goals.
• Providing counseling and emotional support during challenging life transitions, such as bereavement or health crises.

This role primarily serves seniors, a demographic often facing emotional and psychological challenges related to aging, isolation, and changing health. • Master's degree in Social Work from an accredited institution.
• A valid and current state licensure as a Clinical Social Worker (LCSW-C).
• A minimum of 2 years of direct social work experience, preferably in a geriatric or community-centered setting.
• Solid understanding of aging-related issues and a comprehensive knowledge of local community resources.
• Exceptional counseling and assessment skills, with a proven ability to empathize and build rapport with older adults.
• Strong communication and interpersonal skills to facilitate effective collaboration with a multidisciplinary team.
