Key Responsibilities
  • Conduct in-depth psychosocial assessments to evaluate emotional, social, and environmental factors influencing patientsโ€™ health and treatment outcomes.
  • Provide individualized intervention and treatment plans tailored to each patient's unique circumstances and needs.
  • Facilitate discharge planning by coordinating with physicians, nurses, and other healthcare professionals to ensure comprehensive post-hospitalization care.
  • Engage and educate patients and their families on coping mechanisms and resources available to handle life transitions and adjustments related to illness.
  • Utilize diverse therapeutic techniques, including crisis intervention and both short-term and long-term counseling, to help patients address their emotional and psychological needs.
  • Act as a resource for other team members, sharing knowledge and best practices in psychosocial oncology.
  • Participate in the on-call rotation for the hospice center to provide after-hours support and guidance.
  • Deliver educational presentations at conferences and workshops, enhancing community awareness about psychosocial oncology practices.
  • Contribute to community outreach initiatives aimed at promoting mental health resources for cancer survivors.
  • Support the professional development of clinical social workers through supervision and mentorship, fostering the next generation of social work professionals.
Patient Population / Client Focus

This role primarily focuses on patients in hospice care, including those diagnosed with terminal illnesses. You will be instrumental in assisting not only patients but also their families, addressing their psychological and social needs, and aiding in effective coping strategies as they navigate end-of-life issues. Your efforts will directly impact patients' emotional health and overall care experience.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited institution is required.
  • Licensed Clinical Social Worker (LCSW) credentials are mandatory.
  • Minimum of two years of experience in a clinical setting, preferably in hospice or palliative care.
  • Proficient in conducting psychosocial assessments and developing treatment plans.
