Valley Professionals Community Health Center is hiring a Hybrid Patient Monitoring Coordinator (LPN). This position will work out of our South Terre Haute or Cayuga office, whichever is better for the candidate.
Valley Professionals is a Federally Qualified Health Center located in several locations throughout west-central Indiana. Job duties:
Provide professional nursing services and patient care for Valley Professionals Community Health Center. The LPN will follow established nursing processes, including patient assessment, nursing diagnosis, planning, intervention, and evaluation for the Remote Patient Monitoring (RPM) program.
Requirements:
  • Current licensure as a Licensed Practical Nurse
  • CPR Certified.
  • Ability to communicate via Teams or telephone with VPCHC clinic providers and staff.
  • Ability to communicate effectively with patients using a microphone and headset.
  • Basic computer skills, including Microsoft Windows
