Yakima Family Health Services is actively seeking a dedicated Licensed Professional Counselor (LPC) to join our compassionate team. The salary range for this role is between USD 87,000 and 105,000 per year, reflective of your skills and experience.

This position is fully remote, allowing counselors the flexibility to work from home while providing exceptional care to our patients. • Conduct thorough patient intakes focusing on diagnostic and functional evaluations, providing insights and recommendations regarding treatment goals and plans to primary care providers.
• Deliver brief, focused interventions to patients requiring mental health services, ensuring they receive timely and appropriate care.
• Collaborate effectively with primary care providers to manage mental health and psychosocial issues, assisting in identifying which patients may benefit from specialty mental health services.
• Initiate follow-ups to monitor patients' progress and determine whether modifications to treatment approaches are necessary.
• Develop relapse prevention plans tailored to individual patients, helping them maintain stable functioning and emotional well-being.
• Identify “at risk” patients and devise plans to prevent the deterioration of their conditions.
• Document patient progress and diagnostic information accurately within treatment records, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.
• Communicate effectively with primary care providers regarding patients' needs, collaborating to create integrated treatment plans.
• Provide patients with education and self-management skills for their journey towards recovery and mental wellness.
• Consult with clinic management and other team members on behavioral service strategies and evaluation methods.
• Assist patients in complying with medical treatments initiated by primary care providers, including offering advice on coping with medication side effects.
• Uphold the highest standards of ethical conduct in all professional interactions and responsibilities.

The LPC will serve a diverse patient population, including individuals facing various mental health challenges. As a vital component of our care team, you will play a key role in enhancing the physical and emotional well-being of our patients, focusing on fostering rapport and trust to facilitate positive patient outcomes. Your efforts will directly contribute to addressing social determinants of health within the community.

• Minimum of a Master's Degree in Clinical Counseling, demonstrating a solid theoretical foundation in mental health.
• Valid LPC license, confirming your qualifications to practice in the state of Arizona.
• Strong knowledge of mental health assessment techniques, treatment planning, developmental psychopathology, and therapeutic modalities.
• Excellent interpersonal skills to establish and maintain effective relationships with patients, families, and colleagues.
• Proficiency in maintaining accurate clinical records and preparing related reports efficiently.
• A strong sense of professionalism, ethics, and judgment in clinical decision-making.

• Proficient in psychodiagnostics and familiar with current psychotherapeutic techniques and theories.
• Knowledge of community resources available to assist patients and their families effectively.
• Bilingual proficiency in English and Spanish is preferred, enabling broader communication with our diverse clientele.
• Experience in integrating behavioral health with primary care services within a team setting.
