JOB DESCRIPTION
Position: ALF Care Coordinator
Pay Range: $24.00-$27.00 PER HR
Reporting To: Lead ALF Coordinator
Work Type: Remote
POSITION SUMMARY:
The Assisted Living Waiver Program Care Coordination Agency (ALWP CCA) Office Coordinator is responsible for processing the ALWP beneficiary application as per Regulatory Agency and Libertana Home Health Policy.
QUALIFICATIONS:
  1. Knowledge of the State of California Assisted Living Waiver Program Care Coordination Agency Provider Handbook, RCFE Provider Handbook and Title 22 Division 6 for RCFE.
  2. Exercises excellent judgment and has good organizational skills.
  3. Fluent in English.
  4. Possesses effective written and verbal communication skills.
  5. Proficient in the use of computers, electronic medical records, and Microsoft Office.
  6. Ability to establish and maintain good communication and relationships with office, field, and administrative personnel.
  7. Is self-directed with the ability to work with little supervision.
  8. Must be a licensed driver with an automobile that is insured and is in good working order, in accordance with state and/or organizational requirements.
  9. Preferably, has a Bachelor's degree in health care/business administration or a related filed.
  10. Previous Health Care experience preferred.
  11. Background and/or knowledge of developing reports, newsletters, statistics, and information analysis desired.
  12. Possesses excellent analytical skills.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
The following is a representation of the major duties and responsibilities of this position. The agency will make reasonable accommodations to allow otherwise qualified applicants with disabilities to perform essential functions.
  1. Assists Medi-Cal beneficiaries in obtaining access to waiver services, state plan services and other community resources by identifying, coordinating and collecting data required for the ALWP application.
  2. Responsible for the proper completion of all documents required for the State Waiver Program Application as per ALWP and Agency policy.
  3. Reviews and evaluates the ALWP documents necessary for State Waiver Program Application, including Initial Assessments, Re-assessments, ISPs, Physician's Reports Medi-Cal eligibility, etc.
  4. Discusses State inquiries or requests for documentation with Lead ALF Coordinator, ALF Manager, ALF Staff, SNFs and other referral sources.
  5. Responsible for the completion of Patient Charts as per Agency policy.
  6. Establishes and maintains good relationships with RCFE, SNFs and other vendors.
  7. Assists the Agency in maintaining compliance with Federal, State, Local and HIPAA Regulations or Joint Commission Standards.
  8. Assists facilities and ALF Care Coordinators in Habilitation Hours compliance.
  9. Assures that assessments are performed and all necessary documents are completed.
  10. Troubleshoots Client, RCFE, SNF and other vendor issues.
  11. Follow up with RCFE managers, discharge planners and other appropriate client contacts who work there, establishing working relationships and educating them about the ALW Program the services available to them.
  12. Exercises excellent judgement and knowledge of current ALF Care Coordination practices and procedures in alignment with Libertana Policy and DHCS ALWP Policy
  13. Participates in appropriate continuing education as may be required and/or required by your immediate supervisor.
  14. Maintains a comfortable work environment for all employees.
  15. Conforms to all agency policies and procedures.
  16. Respects the patient's and family's rights and property as defined by the federal and state laws.
  17. Driving may be required to geographical areas that are covered by the company.
  18. Knowledge of Confidentiality, HIPAA and healthcare laws and regulations.
  19. Reports Fraud and Abuse.
  20. Knowledge of mandated reporting
  21. Performs other duties as assigned.

PHYSICAL REQUIREMENTS:
  • Stand, sit, talk, hear, reach, stoop, kneel and use of hands and fingers to operate computer, telephone, and keyboard on a frequent basis, up to 75% of the time.
  • Close vision requirements due to computer work.
  • Light to moderate lifting may be required up to 25lbs.
