About the Role:
Skilled Nursing Pharmacy is looking for a knowledgeable and dedicated Long-Term Care Pharmacist Consultant to join our growing team in San Diego, CA. This is an exciting opportunity to make a meaningful impact on patient care by partnering with skilled nursing facilities to optimize medication management and ensure regulatory compliance. If you're passionate about clinical excellence and LTC pharmacy, we want to hear from you!
Responsibilities:
  • Conduct monthly medication regimen reviews (MRRs) for long-term care facility residents
  • Collaborate with physicians, nursing staff, and interdisciplinary care teams to optimize drug therapy outcomes
  • Identify and resolve drug-related problems, including interactions, duplications, and inappropriate dosing
  • Ensure facility compliance with federal and state pharmacy regulations, including CMS guidelines
  • Provide staff education and in-service training on medication safety and best practices
  • Assist in developing and updating pharmacy policies and procedures for LTC facilities
  • Support antibiotic stewardship and quality improvement initiatives within partner facilities
Requirements:
  • Active and unrestricted California Pharmacist (RPh) license required
  • Doctor of Pharmacy (PharmD) degree from an accredited institution preferred
  • Prior experience in long-term care (LTC) or consultant pharmacy strongly preferred
  • Familiarity with CMS regulations, OBRA guidelines, and skilled nursing facility (SNF) compliance standards
  • Strong clinical assessment and medication therapy management (MTM) skills
  • Excellent communication and interpersonal skills for working with multidisciplinary teams
  • Ability to manage multiple facility accounts and work independently in the field
  • Board Certified Geriatric Pharmacist (BCGP) designation is a plus
