Key Responsibilities

The primary responsibilities of the Licensed Master Social Worker include:

• Conducting comprehensive psychosocial assessments of seniors to evaluate their emotional, mental, and social needs, inform care decisions, and identify strengths and challenges.
• Collaborating with clients and their families to create tailored care plans that address individual goals and preferences, incorporating various support services and community resources.
• Providing individual and family counseling, helping seniors navigate life transitions such as health challenges, loss, or changes in living situations.
• Advocating for client rights and ensuring access to necessary community resources, including healthcare services, support groups, and social engagement opportunities.
• Working closely with an interdisciplinary team, including nurses, caregivers, and health professionals to ensure holistic care and effective communication.
• Conducting routine follow-up visits, assessing client well-being and safety, and proactively addressing any emerging concerns.
• Facilitating family meetings to foster communication and resolve any conflicts arising in the care process.
• Educating clients and their families about the variety of community resources available, empowering them to enhance their social involvement and improve quality of life.
• Maintaining accurate records including client assessments, care plans, progress notes, and all relevant correspondence, ensuring compliance with state regulations and ethical standards.
• Keeping abreast of best practices in social work, particularly in geriatric care, contributing to the ongoing improvement of service delivery within the organization.

Qualifications

The successful candidate must possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• A current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of relevant experience in a social work capacity, preferably within a community center or geriatric setting.
• Solid knowledge of social work principles, geriatric issues, and available community resources.
• Exceptional assessment and counseling skills, with the ability to demonstrate compassion and establish rapport with seniors and their families.
• Strong communication and teamwork capabilities to promote effective collaboration with multidisciplinary teams.
• Proficiency in electronic health records (EHR) systems and standard office software applications.

Preferred Experience

Experience working in school-based settings, familiarity with telehealth practices, and a background in crisis intervention are highly desirable. Candidates who have developed or participated in community outreach programs or therapeutic groups targeting senior populations will be recognized as having valuable experience.

Compensation and Benefits

We offer a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes:
