• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, utilizing established methodologies and evidence-based practices.
• Develop and implement individualized service plans that encompass specific goals, tailored interventions, and ongoing progress monitoring.
• Provide counseling and emotional support to individuals and families navigating complex life challenges such as grief, transition, and mental health crises.
• Engage in active collaboration with clients to formulate personalized service plans that address their unique needs and connect them with appropriate community resources.
• Facilitate crisis intervention strategies, conduct thorough risk assessments, and develop safety plans to ensure client well-being during high-pressure situations.
• Work closely with community agencies and organizations to coordinate services and advocate for client needs, ensuring accessibility to vital resources.
• Participate in community outreach initiatives, promoting our agency's services and raising awareness of social issues that affect the populations we serve.
• Contribute to team efforts through participation in case conferences, team meetings, and professional development workshops.

In this role, you will focus on serving clients and families in our Hospice Center, navigating the complexities of end-of-life care and related emotional, social, and spiritual challenges. You will play a pivotal role in ensuring clients receive compassionate support tailored to their needs, which will significantly aid in improving their quality of life.

• A Master’s degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for this position.
• Excellent communication skills, cultural competence, and the ability to cultivate trust and rapport with clients.
• Strong problem-solving skills and the ability to navigate complex human service environments effectively.

• Previous experience in social service agencies or similar settings is preferred.
• Knowledgeable about social and economic justice issues, community resources, and trauma-informed care methodologies.
