  • Develop and implement individualized service plans that encompass specific goals, tailored interventions, and ongoing progress monitoring.
  • Provide counseling and emotional support to individuals and families navigating complex life challenges such as grief, transition, and mental health crises.
  • Engage in active collaboration with clients to formulate personalized service plans that address their unique needs and connect them with appropriate community resources.
  • Facilitate crisis intervention strategies, conduct thorough risk assessments, and develop safety plans to ensure client well-being during high-pressure situations.
  • Work closely with community agencies and organizations to coordinate services and advocate for client needs, ensuring accessibility to vital resources.
  • Participate in community outreach initiatives, promoting our agency's services and raising awareness of social issues that affect the populations we serve.

Patient Population / Client Focus

In this role, you will focus on serving clients and families in our Hospice Center, navigating the complexities of end-of-life care and related emotional, social, and spiritual challenges. You will play a pivotal role in ensuring clients receive compassionate support tailored to their needs, which will significantly aid in improving their quality of life.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for this position.

  • Strong problem-solving skills and the ability to navigate complex human service environments effectively.
Preferred Experience
