Legacy Care Consultants is seeking a passionate and dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team in Dallas, Texas. Work Environment

This position will primarily be remote, allowing for flexibility while you engage with patients and healthcare providers through telehealth platforms and electronic health records (EHRs). Key Responsibilities
  • Conduct comprehensive assessments of patientsโ€™ mental health, evaluating emotional, social, and behavioral factors that impact their well-being.
  • Design and implement individualized treatment plans based on evidence-based practices, considering cultural and personal factors to address the unique needs of each patient.
  • Provide psychotherapy sessions to patients who may be experiencing anxiety, depression, PTSD, or other mental health challenges, utilizing various therapeutic modalities.
  • Regularly monitor patient progress, reviewing treatment effectiveness and making adjustments in collaboration with the healthcare team and feedback from patients.
  • Educate patients and their families about mental health conditions, treatment options, and available community resources to empower them in their care journey.

Patient Population / Client Focus

In this role, you will work primarily with hospice patients facing critical illnesses, many of whom may also struggle with anxiety, depression, and coping with grief. Your skills in providing empathic support will be invaluable as you guide these individuals and their families through complex emotional landscapes, facilitating therapeutic conversations that promote healing and understanding.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience in providing psychotherapy and counseling services to individuals with mental health issues is essential.
  • Exceptional communication and interpersonal skills to build rapport with patients and collaborate effectively with multidisciplinary teams.
  • Strong clinical assessment abilities and experience with evidence-based treatment planning.
  • Proficient in using EHR systems and other healthcare-related software for documentation and patient care coordination.

Compensation and Benefits

The salary for this position ranges from USD 90,000 to 105,000 annually, commensurate with experience and qualifications.
