• Provide individualized psychotherapy and mental health services to patients dealing with a spectrum of issues such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct thorough assessments of patients’ mental health status, considering their emotional, behavioral, and social functioning.
• Develop, implement, and regularly review evidence-based treatment plans tailored to meet the unique psychological needs of each patient.
• Collaborate with a network of healthcare professionals—including psychiatrists, nurses, and primary care providers—to facilitate a unified and holistic approach to patient care.
• Monitor patient progress through coordinated check-ins and adjust treatment plans accordingly, ensuring that feedback from patients and healthcare professionals is incorporated into ongoing care.
• Educate patients and their families about mental health conditions, available treatment options, and community resources, fostering support and understanding.
• Uphold the highest ethical standards in practice, including patient confidentiality and informed consent protocols.

This role centers on a vulnerable patient population within hospice care. • A Master’s Degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• A minimum of two years of experience providing psychotherapy and mental health services to clients, particularly within palliative or hospice care settings.
• Demonstrated exceptional communication and interpersonal skills for effective patient interaction and collaboration with healthcare professionals.
• Strong clinical skills with an ability to conduct comprehensive assessments and create customized treatment plans.
• Familiarity with using electronic health records (EHRs) and telehealth technologies to manage patient information and coordinate care.
• A solid understanding of mental health conditions, treatments, and the specific challenges faced by individuals in hospice care.
• Commitment to maintaining confidentiality and adhering to ethical and professional standards in practice.

• Experience working in hospice, palliative, or end-of-life care environments.
• Skills in conducting family therapy sessions and facilitating group support for grieving families.
• Familiarity with community resources available for mental health support and end-of-life services.
