This role is tailored for a remote workforce, providing the flexibility needed to balance personal and professional commitments while still delivering high-quality care. Although the majority of work is done remotely, occasional in-office collaboration may be required to facilitate team meetings and patient care discussions in our outpatient office setting.

• Conduct comprehensive mental health assessments that integrate the complexities of patients’ physical health conditions, such as chronic pain and substance use disorders.
• Develop and implement individualized treatment plans that blend psychiatric interventions with medical care to support holistic well-being.
• Facilitate both individual and group therapy sessions, employing evidence-based practices like Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) tailored to the needs of each patient.
• Work alongside a multidisciplinary team of medical professionals to ensure seamless coordination of care and optimal treatment outcomes.
• Provide immediate crisis intervention when necessary, demonstrating strong clinical judgment to stabilize and support patients during mental health emergencies.
• Educate patients on the interconnectedness of mental and physical health, empowering them with strategies to manage their overall health effectively.
• Maintain detailed and confidential documentation in accordance with applicable legal and ethical standards to ensure patient safety and compliance.
• Actively participate in interdisciplinary team meetings, contributing insights and recommendations for patient care based on assessment findings.
• Engage in continuous learning to stay abreast of advancements in mental health treatment and integrated care methodologies.

Our patient population primarily consists of individuals facing co-occurring conditions, including mental health challenges exacerbated by chronic medical issues. This role is especially focused on patients with complexities such as autoimmune disorders, chronic pain, and substance use disorders, requiring a tailored approach that honors their unique health profiles.

• A Master's or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• A current and valid license in New York as an LCSW, LMFT, or LMHC.
• Demonstrated experience working with individuals who have co-occurring medical and mental health conditions.
• Exceptional communication and interpersonal skills, enabling effective collaboration with both patients and healthcare providers.
• The ability to maintain professional boundaries while ensuring ethical care and patient confidentiality.
• Familiarity with electronic health record systems and proficiency in using technology to enhance patient communication and documentation.
• A collaborative mindset, with strong capabilities to work as part of an interdisciplinary team.

• At least 2 years of direct experience in a mental health counseling role, preferably in an integrated care setting.
• Experience developing treatment plans that address the complexities of both mental and physical health.
• Proficiency in utilizing evidence-based therapeutic modalities.

The salary for this position ranges from USD 80,000 to 104,000 annually, commensurate with experience and qualifications.
