Key Responsibilities

• Conduct thorough initial assessments, identifying clients' strengths, needs, and risks to formulate personalized treatment plans.
• Provide individual and family therapy sessions, utilizing evidence-based interventions and solution-focused methodologies to promote mental health recovery.
• Maintain up-to-date and accurate clinical documentation, ensuring confidentiality and compliance with state and federal regulations.
• Collaborate closely with a dedicated multidisciplinary team of healthcare professionals to deliver holistic care that addresses physical, emotional, and social needs.
• Educate clients and their families about treatment options, mental health conditions, and coping strategies, empowering them to engage in their recovery journey.
• Continuously research and integrate new mental health practices and trends into therapeutic approaches to enhance client outcomes.

Patient Population / Client Focus

The primary patient population served by our hospice center includes individuals facing terminal illnesses, along with their families, who require emotional and psychological support. As an LCSW, you will address complex emotional responses stemming from grief, loss, and existential concerns while implementing trauma-informed practices to create a safe, supportive space for healing.

Qualifications

• A Master’s degree in Social Work from an accredited institution is mandatory.
• A valid Licensed Clinical Social Worker (LCSW) license issued in the State of Pennsylvania.
• A minimum of two years of clinical experience in a related mental health setting, demonstrating competence in delivering psychotherapy.
• Expertise in solution-focused therapy and familiarity with various therapeutic techniques and models.
• Exceptional interpersonal and communication skills, with a talent for building rapport and trust with clients.
• Strong analytical abilities to assess complex situations and implement effective interventions.
• A collaborative mindset that thrives within a team-focused environment, being open to feedback and continuous learning.

Preferred Experience

• Experience working specifically within hospice, palliative care, or end-of-life settings is highly desirable.
• Knowledge of community resources, support groups, and referral processes to enhance service delivery.
• Previous exposure to trauma-informed care approaches and practices.

Compensation and Benefits
