Job Overview
We are currently seeking a bilingual (Spanish-speaking) professional to join our Care Guide team. The ideal candidate will be fluent, proficient, and confident in both written and verbal communication in English and Spanish. This role is essential in supporting the growing number of members who prefer to engage in Spanish and will play a key part in enhancing the emotional, physical, and social well-being of older adults and their caregivers.
Key Responsibilities
  • Conduct holistic assessments evaluating safety, wellness, social connectedness, and more.
  • Provide case management and build trusted relationships with members and their families.
  • Offer guidance, emotional support, problem-solving, and psychoeducation.
  • Coordinate services and provide ongoing support by phone and digital tools in both English and Spanish.

Qualifications
  • 3-4 years of experience supporting older adults and family caregivers.
  • BSW degree, MSW preferred
  • Bilingual fluency in Spanish and English is required. Must be proficient and confident in both written and verbal communication in both languages.
