The Mohawk Trail Counseling Center is seeking a dedicated Licensed Professional Counselor (LPC) to join our compassionate team on a full-time or part-time basis. Key Responsibilities
  • Collaborate with primary care providers to identify and address the mental health and psychosocial needs of patients, contributing to a comprehensive care strategy.
  • Conduct thorough patient intakes that focus on both diagnostic and functional evaluations, providing clear recommendations for treatment goals and plans.
  • Implement brief, focused interventions for patients requiring immediate mental health support, enhancing their care experience.
  • Advise primary care teams on appropriate patient referrals to specialty mental health services based on clinical assessments.
  • Follow-up with patients to monitor progress and adjust treatment plans as necessary, ensuring that each individual receives the appropriate level of care.
  • Develop and recommend relapse prevention plans, aiding patients in maintaining stability, particularly for at-risk individuals.
  • Coordinate health service delivery related to behavioral health, connecting patients with additional treatment providers as permitted.
  • Maintain detailed documentation of each patient's progress and diagnostic information in compliance with established guidelines.
  • Provide patients with tools for self-management and educational resources, empowering them to actively participate in their recovery journey.
  • Engage in consultation with clinic management and other team members regarding behavioral services and contribute to program evaluation initiatives.
Patient Population / Client Focus

As a counselor, you will work with patients experiencing anxiety, depression, trauma, and other psychosocial issues and will be instrumental in identifying their needs and customizing appropriate care strategies. Qualifications
  • A minimum of a Master’s Degree in Clinical Counseling or a related field.
  • Active Licensed Professional Counselor (LPC) license in Oregon.

  • Proficiency in utilizing DSM-IV diagnosing methods and behavioral therapies.
